Recruit Secure is collaborating with a highly rated Skilled Nursing Facility (SNF) located in the Lansing - Ithaca area that is seeking to directly hire a dynamic Director of Social Services.  The SNF is a 4 -star long-term nursing and rehabilitation facility with about 200+ beds and is dedicated to providing the highest levels of care and commitment to its residents, their families, and their employees. The Director will manage other Social Workers. A salary and full benefits package is included.

Responsibilities:

• Assists in Hiring, Managing and is a resource to the Social Services team.
• Sets goals for the department and communicates clearly and meets regularly with the Social services team
• Directs the Social Services program providing support for residents and families
• Assures that policies and procedures are adhered to for the SNF; makes recommendations, updates, and revisions to policies when and if needed.
• Develops the Social Services plan of care for residents, identifying goals and approaches and participating in inter-department care planning meetings.
• Participates in Quality processes of the SNF
• Counseling residents and family members; facilitating family meetings
• Complete Admissions, Psychosocial Assessments, and Advanced Directives and assures placement in medical records.
• Oversees referrals for health services and facilitates discharge planning process for the department
• Completing all required documentation for medical records; producing monthly departmental reports for the SNF Administrator.

Requirements:

• LMSW certification is a must
• Must have at least 2 years of experience working as a Social Worker within a Nursing or Rehabilitation Facility.
• 1 year of experience in a Senior or Director role
• Should have an excellent working knowledge of available social services and other community resources in the Ithaca area
• Must have excellent communication and social skills.
