Responsibilities:

· Counseling residents and family members; facilitating meetings

· Complete Admissions, Psycho-social Assessments, and Advanced Directives

· Make referrals for health services and coordinate discharge planning

· Completing all required documentation for medical records

Requirements:

· Must have at least 2 years of experience working as a Social Worker.

.  Degree in Social Work.

. Skilled Nursing Home experience is a MUST!

· Should have a good working knowledge of available social services and other community resources in the area

· Must have excellent communication and social skills

· Self- directed but team-oriented
