Recruit Secure is collaborating with a Skilled Nursing Facility located in Winter Park, FL that is seeking an experienced MDS Case Manager (RN or LPN) for direct hire. The MDS Case Manager will be responsible for the timely and accurate completion of the MDS assessments. The successful candidate will have experience with ensuring adherence to systems, policies, processes, State and Federal regulations relating to the RAI processes and reimbursement requirements. A benefits package is offered and salary is based upon experience. 


Some Responsibilities:
•    Ensure that documents are completed for the residents according to Federal and State regulations
•    Coordinate admission process for new residents and assesses new resident in accordance with established systems, processes and standards.
•    Assist with residents from admission to discharge and ensures completion of accurate resident assessments, care plans and discharge plans.
•    Completion of all required documents for Medical Records
•    Coordinate discharge planning in collaboration with Social Workers
•    Actively participate in Medicare meetings and keeps accurate records for ongoing Medicare coverage.
•    Complete MDS using clinical assessments, interviews with resident family and caregiver, observations, and chart reviews ensuring accuracy, completeness, and documentation in accordance with time line requirements.
•    Additional tasks as assigned

Requirements:
•    Nursing Licenses in Florida (RN or LPN)
•    Experience in long-term care
•    Experience with PCC and MDS 3.0
•    Medicare Certification is preferred
