Recruit Secure is collaborating with a beautiful Skilled Nursing Facility (SNF) in Schenectady, NY, that is seeking a dynamic and energetic full-time Director of Social Services for direct hire. The SNF is a long-term nursing and rehabilitation facility with 200+ beds beds and is dedicated to providing the highest levels of care and commitment to its residents, their families, and their employees.  The successful candidate will have at least 3-5 years of experience within a Skilled Nursing Home and a Master’s degree in Social Work. Responsibilities:

• Develop the social services plan of care for residents, identify goals and approaches, and participate in inter-department care planning meetings.
• Participates in Quality processes of the SNF
• Counseling residents and family members; facilitating family meetings
• Complete Admissions, Psychosocial Assessments, and Advanced Directives and assures placement in medical records.
• Oversees referrals for health services and facilitates the discharge planning process for the department.
• Assures that policies and procedures are adhered to for the SNF; makes recommendations, updates, and revisions to policies when and if needed.
• Completing all required documentation for medical records; producing monthly departmental reports for the SNF Administrator.

Requirements:

• Master’s degree in Social Work, LMSW preferred but not mandatory
• Must have at least 3- 5 years of experience working as a Licensed Social Worker within a Nursing or Rehabilitation Facility.
• At least 1 year of experience in a Senior role
• Should have a good working knowledge of available social services and other community resources
• Must have excellent communication skills
