Description

Recruit Secure is partnering with a growing, for-profit healthcare organization that is seeking an experienced Nurse Practitioner (NP) to join its team. This is a direct-hire opportunity based in Williamsburg, Brooklyn, providing telehealth services to residents in multiple Assisted Living Facilities throughout the region.

In this role, the Nurse Practitioner will conduct virtual assessments and collaborate closely with on-site Assisted Living staff members who assist during patient visits. The NP will evaluate residents' medical conditions, develop treatment recommendations, prescribe medications when appropriate, and determine the need for additional services, including Physical Therapy (PT), Occupational Therapy (OT), and other supportive care interventions.

This is a full-time position, on-site in our offices (no travel required), Monday through Friday, from 9:00 a.m. to 5:00 p.m., with some flexibility in scheduling.

Responsibilities

  • Conduct comprehensive telehealth assessments and evaluations of geriatric patients
  • Diagnose medical conditions and recommend appropriate treatments and interventions
  • Prescribe medications and monitor treatment effectiveness
  • Develop, implement, and modify individualized care plans
  • Coordinate with Assisted Living staff and other healthcare providers to ensure continuity of care
  • Identify the need for ancillary services, including PT, OT, and specialty referrals
  • Complete and maintain accurate and timely medical documentation within the EMR system
  • Ensure compliance with Medicare, Medicaid, and all applicable regulatory requirements

Qualifications

  • Current New York State Board Certification as a Nurse Practitioner (required)
  • Active New York State Medicare and Medicaid credentials (required)
  • Experience in Telehealth strongly preferred, but not required
  • Experience working with geriatric and/or Assisted Living populations preferred
  • Strong understanding of Medicare and Medicaid regulations and documentation requirements
  • Proficiency with Electronic Medical Records (EMR) systems and computer applications
  • Demonstrated experience assessing, diagnosing, and treating patients independently
  • Excellent interpersonal, communication, and clinical decision-making skills
