Recruit Secure is collaborating with a highly rated Skilled Nursing Facility located in Jersey City, NJ (180 beds) that is seeking an experienced RN MDS Director for direct hire. The MDS Director is responsible for overseeing the entire patient assessment process and ensuring that assessments and care plans are done accurately and in a timely manner. Additionally, the MDS Director will review and revise individual patient care plans as needed to comply with all regulatory guidelines. Responsibilities:

• Prepare, file, and track electronic paperwork submissions
• Oversee the comprehensive MDS assessment process, ensuring accurate and complete documentation.
• Collaborate with nursing and therapy teams to develop individualized care plans based on MDS assessments.
• Train and educate staff on MDS assessment processes and documentation.
• Ensures that the facility adheres to all regulatory requirements and current standards of practice in relation to the MDS care planning process
• Attend Care plan assessment meetings
• Completing all required documentation for medical records, looking for trends and areas for improvement.
• Collaboratively works with the resident, family, and interdisciplinary team.
• Review clinical records for accuracy and compliance with MDS coding.

• Active RN license from the State of NJ -  a must
• Experience in long-term care and at least 2-year job-related experience in the MDS process.
• Supervisory experience
• In-depth understanding of Federal and State regulations related to MDS
• MDS 3.0 expertise, very familiar with PDPM
• Strong Experience with Medicaid – CMI scoring
• Must have strong familiarity with EMS and MDS software.
• Must be efficient, organized, detail-oriented, and flexible.
