Responsibilities:

• Assures that policies and procedures are adhered to for the SNF; makes recommendations, updates and revisions to policies when and if needed.
• Develop a plan of care for residents, identifying goals and approaches 
• Counseling residents and family members; facilitating family meetings
• Complete Admissions, Psychosocial Assessments, and Advanced Directives and assures placement in medical records.
• Oversees referrals for health services, and facilitates the discharge planning process 
• Completing all required documentation for medical records;

Requirements:

• Master’s degree in Social Work, preferred but not mandatory
• Must have at least 1-2 years of experience working as a Social Worker within a Nursing or Rehabilitation Facility.
• Understand Medicare and Medicaid insurance policies
• Should have a good working knowledge of available social services and other community resources
• Must have excellent communication skills
